When you come to your first appointment, please bring the following items with you:
Photo ID & insurance card(s)
A complete medical history, including current and past conditions, procedures, surgeries, allergies, etc.
A list of all your current medications, including vitamins and supplements
A list of any questions you have for Dr. Friberg
